This error indicates a problem reading or writing XFS filesystem meta-data. It may be caused by a disk failure or it may indicate a SCSI connection/termination/cabling problem.

To resolve this:
Review messages in /var/adm/SYSLOG
Use the hinv command to see the hardware inventory and verify that all the disks on your machine are visible.
Make sure disk cabling termination procedures are followed according to your Owners Guide.
Unmount the file system by selecting the “System” menu from the Toolchest, then selecting the “FileSystem Manager.”
Verify the integrity of the filesystem with xfs_check, for example:
xfs_check /dev/dsk/xlv/xlv0
